• The three following PDF files are respectively the medical certificate, affidavit and the declaration
• These need to be downloaded, manually filled, signed by the concerned authorities.
• The signed forms need to be scanned and later attached while filling the application form
• Soft copy of student’s photo and parent’s signature has to be uploaded while filling the application

• Upload Photo : File Format jpg,jpeg,png,gif Size(Max 2MB)
• Signature of the Student : File Format jpg,jpeg,png,gif,pdf,doc,docx (Max 2MB)
• Signature of Parent Gardian : File Format jpg,jpeg,png,gif,pdf,doc,docx (Max 2MB
• Medical Form : File Format pdf,doc,docx(Max 5MB)
